The “all-on-four” treatment concept basically advocates the use of available remnant bone in atrophic jaws, to allow immediate function and to avoid regenerative procedures that increase both the treatment costs and time, as well as the complications inherent to these procedures.
The protocol uses four implants in the anterior part of complete edentulous jaws to support a provisional, fixed and immediately loaded prosthesis. The two most anterior implants are placed vertically, whereas the two posterior implants are angled at approximately 45 degree. By tilting the two posterior implants, bone to implant contact is enhanced, providing optimized bone support even with minimum bone volume. Additionally, tilting of posterior implants in upper jaw allows for improved anchorage in better quality bone. Tilting also avoids vital structures such as the nerve in the lower jaw and the sinus in the upper jaw and allows better distribution of implants which optimizes load distribution and allows the placement of a bridge with up to 12 teeth, thereby enhancing masticatory efficiency.
This treatment protocol for edentulous jaws have become widely popular among clinicians as well as among patients. High survival rates and a low incidence of complications demonstrate the predictability of implant treatment, regardless of the loading regimen involved. The challenge today is not to prove functionality but rather to develop simple and cost-effective protocols.

Immediate fixed prosthetic solution
